We give discounts under certain circumstances and adjustments in payments can be made if necessary. Please be willing to discuss this openly when we meet for consultation. Please, do not allow money to keep you from caring for your health. This is my mission as well as my vocation and I do not work for free.
You are ultimately responsible for the full professional fee regardless of the amount of reimbursement promised by your insurance carrier. Your insurance contract is between you and your insurance carrier. You are responsible for the performance of your Carrier. Coverage varies and it is essential that you know the terms and limitations of your policy.
To help you submit a claim to your provider, we will provide you with appropriate forms with insurance codes for the services provided.
We understand that there are unavoidable emergencies and inconveniences in everyone's life. If you cannot make it to an appointment, please notify us as soon as possible. Our answering machine is always on. I ask for a minimum of 24 hours notice of cancellation (a call to my answering service at 216-426-1099 is sufficient). If you cannot provide 24 hours notice, a charge will be made for the missed appointment.
